For back teeth we choose composite when the cavity is small to medium and we say so when it is not. Pooled studies of back-tooth composite fillings put annual failure at around 2 percent (Opdam and colleagues, Journal of Dental Research, 2014), so roughly nine in ten are still doing their job at five years; the ones that fail are usually new decay at the edge or a fracture in a very large filling. When a filling would take up more than half the tooth, we recommend an onlay or a crown instead.

How we do a cavity filling in one visit

    Numb.

    Local anaesthetic for anything beyond a shallow surface cavity. We wait until it has taken.

    Remove the decay, and only the decay.

    The chairside camera shows you the cleaned cavity before we fill it.

    Bond and build.

    Composite goes in thin layers, each hardened with a curing light, so the filling bonds to the tooth rather than sitting in it.

    Shape, check the bite, polish.

    You bite on marking paper, we adjust any high spot, and polish the surface smooth.

What we tell every patient before we fill a tooth

The smallest filling is the one done this month. Decay does not stop on its own, and the difference between a filling and a root canal is usually a year of waiting. In the last national oral health survey in India (2002 to 2003), around eight in ten adults aged 35 to 44 had tooth decay, so this is not a rare problem, it is a timing problem.

A filling is not forever. Edges wear and new decay can start beside an old filling. We check every filling at each check-up, and replacing one after years of service is normal, not a failure. Individual results vary with the size of the filling and how the tooth is cared for.

Sometimes a filling is not the right repair. A very large cavity needs an onlay or a crown, and a cavity that has reached the nerve needs a root canal first. We tell you which before we start, with the X-ray on the screen.
